One dungeon, two names

"Northhill Crypt" in the Dark Elf Mage and Grave Keeper descriptions is the same dungeon the game itself calls Spire Hill Crypt — upper level (Lv 11-16) and lower level (Lv 17-19). The achievement text puts the Dark Elf Mage upstairs and the Ogre "in the depths of" the lower level, which lines up exactly with the map config. They are not two separate dungeons.

Where Players Actually Get Stuck

Sorted purely by rate, the 37 fall into five bands, and the bands line up with the zone chain almost too neatly to be coincidence.

One inversion is worth noticing: Extinguish the Crown (1.1%) is more common than Peak of Power (0.5%). More accounts have killed the final boss than have hit the level cap. The API returns one INFERNO_* row at 0.1% and three at 0.0%. The community stats page, however, displays all four at 0.1% — any achievement someone has ever unlocked renders with a 0.1% floor. So which of the four classes holds the 0.1% and which three hold 0.0% is not determinable from public endpoints, and this page deliberately does not guess. Treat all four as a single 0.0-0.1% band.

What is certain is the shape of the wall. Inferno is the third difficulty above Normal and Nightmare, it is where Divine gear lives, and it is gated behind a character that can actually reach the late zones. Clearing it on four separate classes is the real ask behind these four achievements, not four separate fights.
